To request a transcript, please fill out the form below and return it to the Registrar's Office via fax, mail or email. The first transcript is free, however for any transcript after that, there will be a $5 charge.
For each separate request, we will need a separate form. Please fill in the address, email address or fax number you want the transcript sent to legibly. After you fax or email it, please call to pay the fee over the phone with a debit/credit card. Please include payment when mailing your request.
